States must have a non-contributory pension system in place. - In accordance with article 10, paragraph 1, of the Covenant and recommendations 25 and 29 of the International Plan of Action on Ageing, States parties should make all the necessary efforts to support, protect and strengthen the family and help it, in accordance with each society's system of cultural values, to respond to the needs of its dependent ageing members. Recommendation 29 encourages Governments and non-governmental organizations to establish social services to support the whole family when there are elderly people at home and to implement measures especially for low-income families who wish to keep elderly people at home. This assistance should also be provided for persons living alone or elderly couples wishing to remain at home.

Older women also do not have equal opportunities to learn modern information technology, nor the resources to obtain them. Many poor older women, especially those with disabilities and those living in rural areas, are denied the right to education and receive little or no formal or informal education. Illiteracy and innumeracy can severely restrict older women's full participation in public and political life, the economy, and access to a range of services, entitlements and recreational activities.

- Women are fewer in the formal employment sector. Women also tend to be paid less than men for the same work or work of equal value. Moreover, gender-based discrimination in employment throughout their life has a cumulative impact in old age, forcing older women to face disproportionately lower incomes and pensions, or even no pension, compared with men. In general comment No. 19, the Committee on Economic, Social and Cultural Rights recognizes that non-contributory pensions will be required in most States since it is unlikely that everyone will be covered by contributory schemes (para. 4 (b)), while article 28, paragraph 2 (b) of the Convention on the Rights of Person with Disabilities provides for social protection for older women, particularly those with disabilities. Since the old-age pension payable is usually closely linked to earnings during active life, older women often end up with lower pensions compared with men. Furthermore, older women are particularly affected by discrimination on the basis of age and sex, which results in a different mandatory retirement age than that for men. Women should be subject to an optional retirement age so as to protect older women's right to continue working if they wish to and to accumulate pension benefits, where applicable, at par with men. It is a known fact that many older women care for, and are sometimes the sole caregivers of, dependent young children, spouses/partners or elderly parents or relatives. The financial and emotional cost of this unpaid care is rarely recognized.

- Women in rural areas are often disproportionately affected by the lack of adequate health and social services and inequitable access to land and natural resources. Similarly, their situation in conflict settings presents particular challenges with regard to their employment and reintegration, given that it is often exacerbated by the breakdown of services, resulting in food insecurity, inadequate shelter, deprivation of property and lack of access to water. Widows, women with disabilities, older women, single women without family support and female-headed households are especially vulnerable to increased economic hardship owing to their disadvantaged situation, and often lack employment and means and opportunities for economic survival.

Millions of children have been infected and have died and many more are gravely affected as HIV spreads through their families and communities. The epidemic impacts on the daily life of younger children, and increases the victimization and marginalization of children, especially those living in particularly difficult circumstances. HIV/AIDS is not a problem of some countries but of the entire world. To truly bring its impact on children under control will require concerted and well-targeted efforts from all countries at all stages of development.

- However, the ageing world's most important challenge is to ensure the enjoyment of human rights of older persons. It is critical that measures be put in place to eradicate discrimination and exclusion of older persons and to ensure access to services according to their needs. In a statement marking the International Day of Older Persons, the United Nations High Commissioner for Human Rights, Ms. Navi Pillay, pointed to the urgent need for better legal protection of older persons, a growing sector of society that is often most vulnerable and neglected, and she emphasized that "the human rights community has been slow in realizing that the global agenda and the advocacy efforts at the national level can no longer ignore the rights of older persons".

- Formalized long-term care of both types is already prevalent in much of the developed world. In the developing world, traditional social dynamics are also undergoing changes due to various globalizing factors. Families play a steadily less prominent role as primary health-caregiver of older persons, and government institutions and medical professionals are assuming a bigger role in care-giving. The situation is exacerbated in developing countries by the lack of adequate institutional mechanisms and absence of measures to protect the rights for older persons in the context of external and non-family care. Developed countries, where infringements on the rights of older persons also take place, feature only relatively better in developing such mechanisms.

- Due to resource constraints, particularly in developing countries, "best-practice" care may not always be possible. This should not deter States from taking steps to achieve the best possible outcomes for older patients in any given situation. Moreover, it has been acknowledged that personalization of care may, at times, require eschewing of more intensive services and accepting clinical outcomes that are less than the best possible health and function. Although this may still look like an anathema to some medical professionals, a shift in focus from curing to caring, particularly in older population, could be necessary to better meet the patient's personal goals. Presenting patients with options and ensuring that their desired outcomes are achieved through collaborative decision-making should be paramount in the professional-patient relationship.

He further believes that international guidelines and national systems should be developed to regulate practices and ensure that older people are supported in making informed health-care decisions.
