- The impacts of increasingly globalized food chains and the uniformization of diets across the globe have disparate impacts across population groups. As a country transitions towards higher income levels, the burden of overweight and obesity shifts. The poorest segment of the population is at low risk of obesity in poor countries, but in upper-middle income developing economies (with a gross national product per capita of over about US$ 2,500) and in high-income countries, it is the poorest who are most negatively affected. In high-income countries, while the poor bear a disproportionate burden of overweight or obesity, women are particularly at risk because their incomes are on average lower than those of men, and because men in the low-income group often are employed on tasks that are physically demanding and require large expenses of energy. Overweight or obese women tend to give birth to children who themselves tend to be overweight or obese, resulting in lower productivity and discrimination. Thus, socio-economic disadvantage is perpetuated across generations by the channel of overweight or obesity.

- Certain investments can significantly reduce the burden that household chores impose on women. In rural areas, such measures include the provision of water services and afforestation projects to reduce the time spent fetching water and fuelwood. In both rural and urban areas, measures would include the establishment or strengthening of child-care services and care for the elderly or persons with illness/disability. By reducing the time poverty of women, their economic opportunities would expand, since it would be easier for them to seek employment outside the household; access incomes and increase their economic independence, which, in turn, would strengthen their bargaining position within the household. In order for such opportunities to be seized, access to education for girls and life-long training must be improved and societal perceptions of gender roles which discriminate against women must be changed. Improved education and employment prospects are mutually reinforcing, as the demand for education (investment in human capital) will increase in proportion to increase in the demand for a qualified female workforce.

- The right of everyone to the enjoyment of the highest attainable standard of health encompasses access to medical services and the underlying determinants of health, such as water, sanitation, non-discrimination and equality. As access to medicines is an integral and fundamental part of the right to health, Governments and the international community as a whole have a responsibility to provide access to medicines for all. Yet massive inequalities remain in access to medicines around the world, as up to 2 billion people (or one third of the world's population) lack access to essential medicines. Most of them live in low- and middle-income countries, where the needs of persons living in poverty, women, children and undocumented migrants, as well as other marginalized and vulnerable groups who are often discriminated against in terms of access to medicines, are ignored or underestimated.

- The poor often stand to lose most in disaster contexts because they often have to settle on fragile and exposed land that is highly susceptible to the effects of disasters. When a disaster strikes, their pre-existing vulnerabilities are exacerbated, with women, children and marginalized groups bearing the brunt of the impact. After the disaster, the poor often also find their attempts to return to their homes officially denied on the grounds that return would be unsafe, and/or not permissible as they did not have official proof of a right to live there in the first place. This can have dramatic consequences for the livelihoods of individuals, families and entire communities. In the case of conflicts, the displacement and dispossession of specific groups are often deliberate strategies of one group or side in the conflict against another. This can result in the total destruction and/or secondary occupation of their lands and homes, and obstruction of their attempts to return and reclaim what was theirs.

- In core documents emerging from the consultations on the post 2015 agenda, there is little, if any, recognition that minorities experience multiple and intersectional challenges that create vulnerability across a range of areas under consideration. For example, a focus on rural/urban disparities fails to acknowledge that minorities are frequently rural and remote communities with poor access to services and basic needs. Consideration of household wealth disparities rarely makes the link between low income and belonging to a minority. It is often women from disadvantaged minorities who are most affected by poor access to education and decent employment and who suffer multiple discrimination as they are women, members of a minority and poor. When the maps of poverty, access to basic services, gender discrimination, poor housing and population groups are overlaid, minorities stand out as being the most in need. The nexus between minorities, poverty and inequality cannot be ignored.

Rather, the report focuses specifically on the human rights of unpaid caregivers, in particular women living in poverty who provide unpaid care. Other relevant human rights implications of unpaid care work - such as tensions between care and unwanted dependency, abuses against persons with disabilities or older persons, and children's right to receive quality care - are not addressed, and only brief recommendations are made on paid domestic work. The Special Rapporteur hopes that this report will nevertheless encourage broader discussion of the human rights implication of care work.

- The gendered division of unpaid care work is a major reason why women do not enjoy equal rights at work, including fair and equal wages and safe and healthy working conditions. All too often, unpaid care and lack of institutional support for this work traps women between stereotyped gender roles and economic expediency, pushing them into precarious informal employment that is badly paid and involves long hours in difficult conditions. Care responsibilities may also create a barrier to membership of, or active participation in, trade unions for women. As a result, for many women living in poverty with unpaid care responsibilities, work is not empowering but rather a survival necessity.

- Feminized poverty pushes women and girls into domestic work and makes them easy to exploit. Women, who often carry the burden of providing for children, suffer disproportionally from cuts to welfare programmes and essential public services in a situation of economic crisis and budget cuts. In many countries, the collapse of entire agricultural sectors, often linked to inequitable terms of trade, has driven also women and girls into rural-urban or international migrations. With the supply of cheap, desperate labour outstripping demand, power relationships are often so grossly unequal that the degree of exploitation endured by domestic workers depends on the employer's will.

Furthermore, the world's women and girls continue to receive inadequate education when compared to the men and boys from their communities. According to UNESCO, "of the "796 million adults worldwide (15 years and older) who reported not being able to read and write in 2008… two-thirds of them (64%) were women." Being illiterate isolates women, exacerbates poverty, and creates a context ripe for violence.

- The overall targets on increasing access to water and sanitation must therefore be complemented by targets to reduce inequalities. - Foremost in the post-2015 framework debate is the fact that the Millennium Development Goals are silent on discrimination, inequalities and unjustifiable disparities. At least in theory, many of the targets can be achieved without benefiting a single person with a disability, a single person belonging to an ethnic minority, or a single person living in poverty because their focus on average attainments creates a blind spot in the achievement of equality. In practice, the Special Rapporteur has consistently witnessed this reality and she is not alone in her observations. UNICEF has taken a lead role by placing equity at the centre of its development agenda, recognizing that reaching the poorest and most marginalized communities within countries is pivotal to the realization of the Goals. Likewise, UN-Women has also cautioned that, with the target date of 2015 in sight, it is increasingly clear that progress towards meeting many of the Goals is off track. Inequality, including gender inequality, is holding back progress.

Women's access to the judicial system to determine civil claims with respect to divorce, child custody and land inheritance is impeded when excessive fees are imposed. Women living in poverty may also be prevented from filing criminal charges for domestic violence, rape or other forms of gender-based violence because they are unable to afford the fees incurred.

- While globalization has brought greater economic opportunities and autonomy to some women, many others, owing to the deepening inequalities among and within countries, have been marginalized and deprived of the benefits of this process. Globalization should be fully inclusive and equitable. To that end, there is a strong need for policies and measures at the national and international levels, formulated and implemented with the full and effective participation of developing countries and countries with economies in transition to help them respond effectively to those challenges and opportunities. - The Commission notes and expresses deep concern with regard to Millennium Development Goal 1 (eradicating extreme poverty and hunger), that poverty impedes women's empowerment and progress towards gender equality, and that the feminization of poverty persists, and recognizes that significant gender gaps in employment rates and wages persist. The Commission is concerned that owing to, inter alia, socioeconomic inequalities and persistent discrimination in labour markets, women are more likely than men to be in precarious, vulnerable, gender-stereotyped and low paying forms of employment; bear a disproportionate share of unpaid care work; be engaged in the informal economy; and have less access to full and productive employment and decent work, social protection and pensions, which increases their risk of poverty, relative to men, particularly if they are living in households without other adult earners. It further notes that discriminatory norms contribute to women's and girls' greater vulnerability to extreme poverty, food insecurity and malnutrition and that girls and older women each face different and particular challenges. The Commission notes that current poverty measures do not adequately reflect women's vulnerability to poverty, owing to inadequate data, inter alia, on income distribution within households. The Commission is further concerned that the targets on hunger also remain unmet, with adverse consequences for the health, livelihoods and well-being of women and girls. It notes the importance of food security and nutrition for achieving goal 1 and the need to address gender gaps in the fight against hunger, and recognizes that insufficient priority is given to addressing malnutrition in women and girls.
